Community Connector – MIALS

Community Connector Reports To: Director of Programs and Services

Type: Part-Time (10 hours per week) July 2026 to March 2027
(possibility of extension based on funding)

Salary Range: $27 – $33/hour (based on qualifications)

Position Summary:
The Community Connector is a specialized role within a Social Prescribing model. You will work with older adults (55+) who are at risk of social isolation, frailty, or chronic disease. Working from a strengths-based, trauma-informed, and culturally safe lens, you will meet with seniors to discuss holistic wellness, aging well and to develop a wellness plan. You will assist clients to navigate complex health and social systems, linking them to non-medical supports such as exercise programs, meal delivery, housing advocacy, and social groups. Your ultimate goal is to empower seniors to age safely, with dignity, in their chosen community.

Key Responsibilities:
1. Intake, Assessment & Co-creation of Wellness Plans (40%)
Conduct community-based wellness assessments to identify health assets and risks.
Utilize validated tools to understand seniors’ unique strengths, goals, and cultural contexts.
Co-develop personalized Health and Wellness Plans that mitigate health risks including physical, mental, emotional and spiritual. Provide coaching support to help client engagement with wellness activities.

2. Community Connection & Asset Mapping (30%)
Build and maintain a database of community wellness assets and mobilize informal senior wellness networks.
Communicate with seniors, caregivers and wellness professionals to reduce barriers to positive solutions to ageing in community.
Attend community events to monitor wellness and engage more seniors in wellness planning.

3. Collaboration & Systems Navigation (20%)
Participate in Communities of Practice with other Connectors and healthcare partners to align social and medical care.
Work across sectors—housing, income assistance, and justice—to advocate for systemic changes that reduce health barriers for seniors.

4. Documentation, Reporting & Evaluation (10%)
Track client progress and project outcomes including service statistics.
Collect client stories to illustrate the human impact of social prescribing

Qualifications & Assets Education & Experience:
• Diploma or Degree in wellness, human services or community development. Or equivalent lived or professional experience.
• Minimum 2 years working directly with adults, particularly with a health and wellness focus.
• Demonstrated understanding of physical, mental, emotional and spiritual wellness activities.

Core Competencies:
• High emotional intelligence, patience, and non-judgmental listening skills.
• Relationship building with diverse individuals and members of diverse groups. • Skills in conflict resolution, and safeguarding vulnerable adults.
• Technical skills in wellness and data management.

Logistical Requirements:
• Willingness and means to travel to community events.
• Clear Criminal Record Check (Vulnerable Sector).
• Personal safety awareness.
