Community Connector & Social Prescribing

Our Community Connector, Tim, welcomes fully confidential and non judgemental conversations about your health and wellbeing. He can help you by giving options and supporting you to make choices on how to improve your overall wellbeing utalizing a variety of community knowledge and reasources through helping you to make:

  • positive changes in your life
  • talk to about how you are feeling in a non-theraputic setting
  • help with finding information about housing, benefits and financial issues
  • help guide you through technology challenges
  • becoming more involved in the community
  • becoming more physically active

Anyone who is 55+ living on Mayne Island SḴŦAḴ who could use a little extra help with…

  • life changing events such as retirement, bereavement, changes in health and independence
  • getting out and about in nature and/or the community
  • guidance to find information to help with finances, housing and accessing benefits
  • creating healthy lifestyle choices
  • loss of confidence / purpose
  • social isolation / loneliness
  • emotional wellbeing
  • caregiver stress

Step 1
You can contact your Community Connector directly through 
our website or give us a call at 250-213-6181 to find out more. A health professional can also talk to you about how Social Prescribing services may help, and send a referral to your local Community Connector on your behalf.
Step 2 
Your Community Connector will reach out to have a conversation with you, either in-person, over the phone, or by a method of your choosing.
Step 3 
Your Community Connector will spend time with you exploring what activities, services and local support is available that could support your health and wellbeing.
Step 4 
Together you will identify goals and create a personalized plan to achieve them.
Step 5 
Your Community Connector will connect you with local services and follow-up with you on your personalized plan to see how it is unfolding for you. Making any adjustments to your needs that still need to be met.
